The Uniform Hazardous Waste Manifest (EPA Form 8700-22) is required for off-site transport of RCRA hazardous waste under 40 CFR Part 262. All waste generators that transport or offer for transport hazardous waste off-site must use this manifest. The manifest must accompany the waste shipment and copies must be retained by the generator, transporter(s), and TSDF.

Who is required to use EPA Form 8700-22?
Any generator that ships RCRA hazardous waste off-site to a treatment, storage, or disposal facility (TSDF) must prepare a Uniform Hazardous Waste Manifest. This includes Large Quantity Generators (LQG), Small Quantity Generators (SQG), and in some states, Very Small Quantity Generators (VSQG). Requirements are in 40 CFR Part 262. State regulations may add additional fields or require state-specific forms to accompany the federal manifest.
What are EPA waste codes — D, F, K, P, U?
D Characteristic wastes (ignitability, corrosivity, reactivity, toxicity) — e.g. D001 ignitable, D002 corrosive, D003 reactive, D004–D043 toxicity. F Non-specific source wastes from common industrial processes — e.g. F001–F005 spent solvents. K Specific source wastes from particular industries — e.g. K001 wood preserving. P Acute hazardous discarded commercial chemical products. U Toxic or hazardous discarded commercial chemical products. You can enter multiple EPA codes separated by spaces on each waste line.
What is a DOT Proper Shipping Name and where do I find it?
The DOT Proper Shipping Name is the official designation from 49 CFR Part 172, Hazardous Materials Table (49 CFR §172.101). For hazardous waste, the entry must include "RQ" if a reportable quantity is present, the UN/NA number, the proper shipping name, hazard class, and packing group — e.g. UN1993, Flammable liquid, n.o.s. (contains acetone), 3, PG II. The tool includes a built-in lookup table for common waste descriptions; you can also type the name manually. Always confirm against the current DOT HMT.
What do the management method codes mean?
Treatment codes (T): T06 = Incineration/thermal treatment, T11 = Liquid injection incineration, T25 = Land treatment, T26 = Thermal treatment other, T28 = Solvents recovery, T35 = Solidification/stabilization, T37 = Biological treatment, T38 = Ion exchange, T39 = Neutralization, T40 = Oxidation, T45 = Carbon adsorption, T50 = Other treatment. Storage codes (S): S01 = Container storage, S99 = Other storage. Disposal codes (D): D79 = Subtitle C landfill, D80 = Surface impoundment, D81 = Injection well. Is the e-Manifest system replacing paper manifests?
Yes. EPA's e-Manifest system (RCRAInfo) is the national electronic manifest system and is now the primary method for manifest submission. All TSDFs must use e-Manifest. Generators and transporters can use paper, hybrid, or fully electronic manifests. This tool generates a paper-format PDF suitable for situations where a paper or hybrid manifest is used, or as a draft before electronic submission.
